What Actually Happens in a First Counselling Session
Most people put off their first session because they do not know what they are walking into. Here is the honest version — what gets asked, what does not, and what you are allowed to hold back.

The gap between deciding to get help and actually booking is often months long. When we ask people what held them up, the answer is rarely cost or time. It is not knowing what happens in the room.
So here is the unglamorous version.
Before you arrive
You will have filled in a short intake form covering the practical things: contact details, medical aid if you are using one, anything a counsellor should know for safety reasons. It exists so the first session is not spent on admin.
Online sessions run over a secure video link. In-person sessions happen in a private room. Either way, nobody else is in the session unless you have asked for them to be.
The first ten minutes
Your counsellor will explain confidentiality before anything else — what stays in the room, and the narrow legal exceptions where it cannot. Those exceptions are essentially about immediate risk to life. They are not about judging your choices.
If your sessions are being recorded for clinical notes, that is explained here too, and it only happens with your consent.
The middle: mostly you talking
The first session is largely about orientation. Your counsellor is trying to understand what brought you in now — not just what is wrong, but why this month rather than last year. That question tells them more than a symptom list does.
Expect broad questions rather than a rigid script:
- What is going on at the moment that made you reach out?
- How long has this been building?
- What does an ordinary week look like for you?
- Who is around you, and are they helping?
- Have you had counselling before, and how did that go?
There is no requirement to tell your whole story in the first hour. People often arrive believing they have to explain everything or they are wasting the session. You are not. Starting with the last two weeks is completely acceptable.
